We gladly file insurance forms for you and accept payment from them, however for most PPO plans we file as an out-of-network provider. Before treatment begins, we will estimate your out-of-pocket costs based on the information provided by your insurance company, and we expect payment of your portion at the time of your appointment. Please remember that you are responsible for any amounts not covered by your insurance company.
